Recently I signed up with a host for year ahead and after that they stopped responding all my support requests. So, I'm currently in search of a realiable hosting service for three
of my domains ( and few more are coming soon). I'm commited to a long
term contract. However, before I go for it I need to find the service
which matches to all of my requirements below.

If you are a host provider and can match my requirements or know a good service please let me know.
Thank you in advance.

- Shared ( virtual) hosting on Windows platform. Preferably MS certified. Unix will be considered too.

- Fast network connections without any forced traffic limiting
( such as limiting concurrent connections, etc.)

- Setting an account within 12 hours ( 24 max.)

- $5-$6/mo. ( up to $10). No long term contract. Monthly billing only.
No set up fee. No hidden charges. 30 days (no less) money back guarantee.

- Good (or excellent) customer support by telephone ( preferably) or e-mail with response
time around 12 hours ( 24 hours max.)

- Min. 50 MB of disk space (more is OK), unlimited bandwidth ( min. 20-30 GB). I use less than
that but want to have more just in case.

- CGI directory, Perl, ASP support

- Custom error page

- Full support for http://domain.com and http://www.domain.com ( no aliasing
from domain.com to www.domain.com)

- min. 5 e-mail accounts with autoresponders and forwarding capabilities.

- Support for MS Access database ( MySQL will be considered)

- FTP access 24/7.

- Access to log files for using with third party log analizers
(log files do not have to count toward disk space)

- Dedicated IP address.

- Password protected directory

- Web based control panel for mail and other site configuration.

- Server Side Includes

- Other feartures are optional.

Sorry, but your request is very unrealistic. Maybe the reason why your current hosting company is not responding to e-mails and the like is because they don't have the resources to answer them, and their business plan is failing. You are asking for WAY to much, and I hope that you would come down to earth a little more with your request. How can you expect to pay $5 month for 20-30gb bandwidth?
